Starting Sunday, Oct. 10, 2021, lab results will be automatically release to you and your provider. It is possible a result may appear in the portal prior to being reviewed by your Doctor. You will be notified via email once your Doctor has completed their review, and may want to hold questions until that time.

As an existing My Care Plus user, you can request a new password. Enter your Email Address associated with your account, Date of Birth and then click Submit. If you are a caregiver with authorized access, please provide your Email Address and the patient's Date of Birth.

You will receive an email with instructions on how to reset your password.
